Unused concept art from Scott Derrickson’s original plan for Doctor Strange in the Multiverse of Madness reveals the villain, Nightmare.
Marvel Studios’ Doctor Strange in the Multiverse of Madness was originally supposed to be directed by Scott Derrickson, who was at the helm of the 2016 origin story. However, Derrickson left the project over creative differences, and original Spider-Man trilogy and Evil Dead director Sam Raimi took over the reins on the sequel, while Michael Waldron wrote the film’s script. As it turns out, Derrickson’s original plans included the villain, Nightmare.
In the Marvel Comics, Nightmare is a demon-lord that rules over his own realm within the Dream Dimension known as the Nightmare World, which is a manifestation of the collective psyche of humanity, in which people’s dreams manifest their own lives. Some of his abilities include feeding off of dreams of living people during their slumber, as well as trapping people into his realm. Based on the premise of the Doctor Strange sequel, it makes sense as to why Scott Derrickson originally wrote the character into the story.

Early character designs from Scott Derrickson’s version of the Doctor Strange sequel showcase how Nightmare could’ve looked like. Here, we get a sense at how Derrickson would’ve pushed his own take on the horror genre infused within the film. Although Sam Raimi is notable for his horror roots, perhaps Derrickson’s could’ve been a bit less campy and more just purely scary:
